Surgical Increasing of Penis Girth Using Fat Cells
An innovation in surgical penis enlargement is the injection of fat from other parts of the body (generally liposuctioned fat from the abdominal wall or thighs), into the penis, under the skin of the penile shaft. This is done to increase the thickness (the girth but not length) of the penis.
There are many surgeons who refuse to perform such a surgery because of its significant drawbacks. One of them is that the size of the penis head cannot be increased and this will result in a visually odd penis. Additionally, the newly achieved girth of the penis can often disappear since the fat cells can be reabsorbed back into the body. Moreover, this re-absorption can be uneven, resulting in clumping and unsightly looking penis.

Another method for surgical penis girth enhancement involves silicone injection into the penis and scrotum. Although this technique can increase the girth of the penis significantly (over 9 times), it is effectively irreversible and may have some serious side effects such as loss of sensation, deformation and even inability to perform sexual intercourse.
Surgical Increasing of Penis Length by Cutting the Suspensory Ligaments
Penile lengthening procedures are quite different from the penis girth increasing procedures. One of the most common techniques for lengthening the penis is to cut the basal penile ligament (the suspensory ligament).
However, the suspensory ligament does have its role - it serves to keep the penis pointing in upward direction during erection. This means that after this surgery the erection may point down and even when the penis hardens, it will no longer be truly erect, but will hang instead. In addition to that, the actual results of this surgical enlargement are controversial since there are men who report no lengthening at all.
Surgical Penis Enlargement with Penile Implants
Another method for penile enlargement is the replacing of the Corpora Cavernosa with penile implants that are inflatable by means of an implanted pump that can be manipulated by hand to achieve erection. Usually this surgery is performed for men suffering from complete impotence. Therefore normally the replacement cylinders are the same size as the Corpora Cavernosa, but naturally larger ones are used when penis enlargement is the desired result.
Again, there are certain drawbacks of this penis enlargement method, such as an uncomfortable stretching of the penile tissues which can result in a number of complications.
